What’s new or changed for the care team

Updates to the Care Manager Workspace

So that you can quickly search for the client you are interested in, there is a quick client search added to menu bar. You can now request emergency access to a client from the search results.

Updates to the Care Plan Summary Report

For compliance and readability, the layout of care plan summary report has been improved as follows:

  • Increased column width and line spacing
  • Actions are grouped under their associated goal
  • Assigned to information displays for care team actions, client actions, and actions assigned to a care team role.
  • If configured, the added reason for actions displays.
  • More historical program enrollment information displays.
  • Source information for medications and conditions displays.

Adding a goal to a client’s plan without associating it with a program

So that you can add a client’s goal before the client is enrolled on a program, or so that you can add a goal to a client who is not participating in a specific program, you can now add a goal without associating it with a program.

Associating barriers with services

Barriers can hinder or prevent a client from using a service. A Barriers panel is added to the Services page so that you associate a client barrier with a service, remove a barrier from a service and view barriers that are associated with a service.

Associating goals with services

You can associate services with a client’s goal by adding the goal to the service. A Goals panel is added to the Services page so that you can add goals to a service, remove goals from a service, and view goals that are associated with a service.

Task filtering

So that you only see the tasks that you want to see, you can now filter the Task lists. The following predefined filters are available:

Filter Description
High Priority Tasks Shows all open high priority tasks that are assigned to me and to my care team roles
My Due Tasks Shows all open tasks that are assigned to me and are due today
My Open Tasks This is the default filter. Shows all open tasks that are assigned to me.
My Overdue Tasks Shows all open tasks that are assigned to me with a due date before today
My Unscheduled Tasks Shows all open tasks that are assigned to me that have no due date
My Closed Tasks

Shows all tasks that are assigned to me with a closed status

Open Tasks Assigned to Others Shows all open tasks created by me that are assigned to someone else
Closed Tasks Assigned to Others Shows all tasks created by me that are assigned to someone else and that are now closed by me or by someone else
Unassigned Tasks Shows all open tasks that are assigned to all my care team roles

In addition, you can create filters based on Category, Priority and Due date.

Notes filtering

So that you only see the Notes that you want to see, you can now filter the Notes list. The following predefined filters are available:

  • Recent Notes (Last 5 Days)
  • Touchpoints
  • Alerts Notes
  • Barrier Notes
  • Open Notes
  • Cancelled Notes

In addition, you can create your own filters based on Notes, Subject, Type, and Latest Update.

Adding attachments

You can now add attachments in .tif or .tiff formats. For a full list of the file types that you can upload see, File type restrictions.

Working with client data

Communications Opt out

You can now indicate if a client has opted out of being contacted via specific phone numbers, text, email, or address. This preference can be back dated.

Filtering data

So that you can easily see the information that you are interested in, you can sort data lists as follows:

    Date, sorts on date in ascending or descending order based on your selection.
  • Description, sorts alphabetically in ascending or descending order based on your selection.
  • Updated On, sorts on date in ascending or descending order based on your selection.

Each data type can be independently sorted.

Conditions data

There are new out of the box coded values for Conditions.

Client medication

So that you have an accurate picture of the client’s medication, you can now identify the medications the client is currently taking or using. As not all records have consistent date fields populated, the date displayed for a medication depends on the availability of the date value. The application checks date fields in the following order until a populated date field is found:

  1. Start Date
  2. Ordered Date
  3. Prescription Date
  4. Fill date
  5. Last fill date
Viewing and filtering of the client’s medication list

Filter Description
Active medication This is the default filter. Shows all medication records that have an active or unknown  Status with an end date in the future or today.
Inactive medication Shows all medication records with a status that is not active or unknown with an end date in the past
Currently taking Shows all current medication that the client is currently taking.

What’s new or changed for supervisors

The unassigned client list

When you are assigning clients for to care managers or when you are rejecting referrals for active clients, you can now filter the unassigned client list based on client priority (High, Medium, Low, Not Set), or unassigned day ranges.

In addition, you can further filter the displayed client list by Name, Referred On date, Cohort, Referral Reason, Risk Name, or Risk Category.

In addition, you can sort the list based on the following:

  • Name: sorts the list alphabetically in ascending or descending order depending on your selection.
  • Referred On: sorts the list by date in ascending or descending order depending on your selection.
  • Priority: sorts the list by High, Medium, Low, Not Set priorities in ascending or descending order depending on your selection.
  • Cohort: sorts the list alphabetically in ascending or descending order depending on your selection
  • Referral Reason: sorts the list alphabetically in ascending or descending order depending on your selection.

What’s new or changed for administrators

Adding attributions to questionnaires

In the same way that you can add and edit attributions to assessments, you can now add attributions to Questionnaires. This allows you to ascribe the questionnaire to the organization that created it and that owns the intellectual property and copyright. For example, for questionnaires that have been developed by IBM, you can ascribe the questionnaire to IBM.

What’s new or changed for security administrators

Audit reports

Security administrators can now obtain audit reports that show who has access to which clients’ PHI. You can request audit reports via IBM Support.

Defects resolved

  • Error uploading attachments that have filenames with more than 64 characters. Note: You can now upload files with filenames of up to 250 characters. (198581, SR 1300)
  • Attachment file types of .tiff and tif can now be uploaded. (197001, SR 1295)
  • Error message displays on a custom data summary card when a configured value set attribute is left blank, for example if a codetable item is blank. (197907, SR1320)
  • Internal server error when you enter custom data. (190509, SR1256)
  • Expanding the unassigned card on the Supervisor Workspace gives an error. (198577, SR 1312)

Known issues

  • When you apply a custom filter to return all of your tasks that are due today, all tasks assigned to you that are due today and all tasks that were created by you for other users that are due today are displayed. (202982)
  • When you sort data types by description, descriptions that start with whole numbers are not sorting correctly. (199039)
