Engaging clients in their own care

If your organization licenses the IBM Watson Care Manager Connect Individuals add-on, you can invite your clients to register to use the Connect application.

Clients can access the application on any browser from any Internet-enabled device.

Using the Connect application, clients can securely manage their health goals. They can view their active and completed goals and actions, and can mark actions as complete when they are finished them. Clients can browse service providers and shortlist the providers that best suit their requirements.

Updates that are made by a client using the service are saved to Watson Care Manager. You can view the changes that are made by a client and their shortlisted providers in the care team application.

Deleting a client

As clients may request to have their personal data erased from Watson Care Manager, for example, under data privacy regulations or as organizations may have compliance workflows that require the erasure of personal data, you can now delete a client in Watson Care Manager.

Before you delete a client, you must follow the steps in the Delete Client checklist.

When the client is deleted,

  • information relating to that client is no longer accessible in the Watson Care Manager application.
  • the client’s data is removed from all processing in Watson Care Manager.

You cannot undo the delete client operation.

IBM Watson Care Manager Community Service Referral

To improve your user experience, the following improvements have been made to the Watson Care Manager Community Service Referral add-on:

To help you avoid issues with budget spending, now if you are using Watson Care Manager Community Service Payment add-on, you can no longer accept a provider’s offer if the client’s budget has not been approved. When you are viewing an enquiry, the fields are updated to improve clarity. The fields displayed in the collapsed header are,

  • Service name
  • Sent on – the date the enquiry was sent and by whom
  • Units – the number of units required and unit type
  • Frequency – the frequency of service required, for example, Weekly, On a Tuesday & Thursday
  • Response Required by
  • Expected Start Date
  • Expected End Date
  • The Total Offered Rate field is removed from the Enquiry Timeline, as it was not clear what this value represented. When you are creating an enquiry, the fields on the Create Enquiry page are reordered into a more logical grouping. When you add a provider to the shortlist, the shortlist icon is changed to the word ‘Shortlisted’.

    IBM Watson Care Manager Community Service Payment

    To improve your user experience, the following improvements have been made to the IBM Watson Care Manager Community Service Payment add-on:

    • The New budget link is now be available on the Care plan workspace.
    • If you do not have the budget approver role, the Cancel approval request action is now disabled.
    • The Weekly Budget Summary tooltip is renamed to Highest Weekly Spend.
    • The Weekly Budget Information tooltip is renamed to Existing Budget.
    • An informational icon now displays on reopened budgets to inform the user if there is an existing closed budget with a future end date.

    Managing client referrals

    So that you can quickly create referrals for clients, now in addition to being able to create a referral from the client summary, you can now search for a client and create a referral.

    Deactivating clients

    When a client is being deactivated because the client has graduated from their program or completed their care plan, you can now select a new deactivation reason Graduated/Completed.

    Summary

    Your Watson Care Manager administrator can now configure the cards that display on the client summary view based on care team role, so that you see the information about a client that is most relevant to your role.

    Viewing unscheduled and overdue tasks

    So that you have an easily accessible overview of all tasks that are unscheduled or overdue, the Tasks list page opened from the respective worklist displays with the unscheduled or overdue tasks filter selected by default.

    Filtering updates

    When you filter any list in the application, the filters that you are using are displayed across the top of the list.

    Recording if your client has no known allergies

    You can now record that your client has no known allergies. So that you can be specific on the type of allergies in question, you can choose from several types of allergies, for example No Known Drug Allergy, No Known Food Allergy, or No Known Latex Allergy.

    Integrations with IBM Explorys EPM Registry

    If your instance of Watson Care Manager is integrated with EPM Registry, when you manually register a client, a request is made to EPM Registry to check if the client is also registered there. If the client is registered in EPM Registry, the client’s record in EPM Registry is matched with the client’s record in Watson Care Manager and data about the client that is stored in the external system is sent to Watson Care Manager. This is an overnight process.

    In addition, if configured, updates to demographic data made in Watson Care Manager can be shared with EPM Registry.

    Reviewing clients’ current allergies

    A client’s list of current allergies can include allergies that they no longer have. As part of an allergy review with the client, a user can now flag if a client currently has a particular allergy or not by selecting a Currently Active Yes or No value beside the allergy name.

    Manually recording of medications, conditions, and allergy data

    So that you can record all medications, allergies and conditions that a client has, you can now manually record a value when recording Medications, Allergies, and Conditions data for a client when the value that you want is not available when you search for it.

    Conditions integration with terminology services

    When you record a client’s conditions manually, you can now search for a condition using the SNOMED coding system. You can search for a condition by name in Watson Care Manager. Watson Care Manager searches SNOMED for the condition. You are presented with possible matches and can choose the appropriate one for your client. /p>

    New name for the CareConnect application

    The CareConnect application is renamed to Connect.

    What’s new or changed for supervisors

    Active or inactive referrals

    So that you have an overview of all the open referrals for your organization unit, referrals for both active and inactive clients are consolidated into the Unassigned Referrals list. You can also activate an inactive client and assign their referral in a single step.

    It is also now possible to filter the lists by client status.

    Filtering updates

    What’s new or changed for administrators

    Configuring the summary view by care team role

    You can configure the client summary view based on care team role, so that people with different roles in your organization see the client summary information that is of interest to that role. If a user has multiple care team roles, the summary view displays the default summary view for a client.

    Important: Do not configure client summary cards by role as configuring summary cards for a role is not supported in this release due to an issue with the reordering of summary cards for a role. If an administrator configures and reorders client summary cards for a role, the client summary that displays for a care team member with that role does not reflect the reordering. The summary cards display in the default card configuration.

    This issue does not affect the Default Client Summary Configuration, and there is no impact to existing Client Summary configurations.

    Administering Connect Providers user accounts

    The CareConnect application that us available with the Watson Care Manager Connect Providers add on is renamed the Connect application.

    What’s new or changed in reporting

    Notes and Touchpoints

    Touchpoints and Notes data sets are now separated into their own folders. Note: The Touchpoint and Notes tables that are within the Individuals In Programs folder are now deprecated but will be retained for a number of releases. You should use the tables in the new folders for existing or new reports that contain Touchpoints or Notes data..

    New clinical data sets

    Clinical data sets for a client’s Allergies and Vitals are added.

    New data attributes

    There is new data attributes available for the Laboratory Tests data set.

    Defects resolved in this release

    Watson Care Manager

    • You cannot remove a care team member that does not have access to Watson Care Manager from a client’s care team. (224688, TS000155165)
    • Coverage data is not displaying in Watson Care Manager as expected. (222196, 222755, TS000127903, TS000103054)

    Watson Care Manager Connect Providers

    • When using Internet Explorer 11, when you try to add a Service from the Services tab on the CareConnect application, a blank page displays. When you view services on the Services tab, the services names do not display. To avoid this issue, use one of the other browsers documented in the Watson Care Manager System Requirements. (223219, 223218)
    • When using the CareConnect application, if you update your organization’s address and your organization’s details in the one user session, an error displays. To avoid this issue, reload the browser page between updates. (222072, 661)
    • Medications list is not sorting alphabetically. (216547, TS000073966, TS000073968)

    Known Issues

    Watson Care Manager

    • Reordering of summary cards configured for a role is not supported for use at this time. If an administrator configures and reorders client summary cards for a role, the client summary that displays for a care team member with that role does not reflect the reordering. The summary cards display in the default card configuration. This issue does not affect the Default Client Summary Configuration, and there is no impact to existing Client Summary configurations. (226520)
    • The reporting application is treating Allergy names as case sensitive, for example, when you generate a report of the number of clients with specific allergy types, allergy names in different cases are being considered as two different allergies. (224396)
    • When configuring a Share the Care Plan template (Care Plan Summary template), the expected validation does not display if you reorder goals and actions. (224609)
    • All Tasks not displaying. (220044, TS000107784)
    • When a 3 letter search is performed when adding Conditions, Medications, or Allergies in Watson Care Manager, a blank page displays if too many results to display are returned. To avoid this issue, refine your search criteria. ( 217057, TS000047126, TS000055247)
    • For systems integrated with EPM, the alert triggered when EPM returns Risk Scores to Watson Care Manager may contain some inaccurate information. The alert indicates that the client has been matched on the EPM system and clinical data has been received by Watson Care Manager. However, this may not be the case as Risk scores are automatically generated for any client received by EPM even if the client was not matched on EPM. (223580)

    Watson Care Manager Connect Providers

    • If you close an active or pending provider account you will not be able to create a new provider account using the same email. (222962)
    • If you use Watson Care Manger Connect Individuals or Watson Care Manager Connect Providers, the administrator must configure your organization name in Watson Care Manager. ( 223689, 224127)
    • The onboarding emails sent to a users of Connect Providers and Connect Individuals contain set password links. These URL links display as plain text in some email clients. If you encounter this issue, cut and paste the URL into a web browser to access the link. (223222)
    • When you view an enquiry in the Connect Providers application, the date fields are not displaying according to your browser settings. Date fields display as DD/MM/YYY. (223535)
    • There is no indicator that a CareConnect session has timed out. Timeout occurs after 30 minutes. If your session times out, you can navigate between the application pages but content may not be displayed correctly and you will not be able to edit or add content. If this happens, reload the browser page to display the login screen and sign into the application again. (222164)
    • If the Watson Care Manager administrator adds new unit type for a service or modifies an existing unit type after a provider has logged into the CareConnect application, the configuration changes are not reflected in the CareConnect application. The same issue exists with language configurations. (221039)
    • In the CareConnect application, you are incorrectly allowed to save a rate with the same unit type and for the same time period as a rate that is already saved. This makes it unclear to the care team member which rate applies for the time period when they are selecting the service. (223285, 500)

    Watson Care Manager Connect Individuals

    • The Show Password link on the Connect application sign in page is not working. (224955)
    • When you are sending an onboarding request to a client inviting them to use the Connect application, the message that displays if you forget to select an email address incorrectly refers to the MyHealth application instead of the Connect application. (224949)
    • If you use Watson Care Manger Connect Individuals or Watson Care Manager Connect Providers, the administrator must configure your organization name in Watson Care Manager. (223689, 224127)

    • Clients that have completed registration to use the MyCare application can still access the application when they are deactivated in Watson Care Manager. (218091)
